• Address your request to the Virginia Office of Vital Records. Simply write:
  • Name of deceased
    
  • Date of death
    
  • Place of death
    
  • Relationship to the deceased
    
  • Reason for the certificate
    
  • Area code + daytime phone number
• Mail the above request, along with any applicable fee, and a copy of your government issued photo identification to:

Notes:

• Per Virginia state law, death records are available to the public 50 years after the event.
• Make sure to pay for your fee with check or money order: cash will not be accepted.
