• Vermont provides free death records for genealogical purposes (not certified):
• Download this form from the Vermont Office of the Secretary of State.
• Fill it out, then email the completed form as an attachment to vitals@sec.state.vt.us
• If you need a certified death record, download this form from the Vermont Office of the Secretary of State.
• Complete the above application and mail it, along with any applicable fees and a copy of your government issued photo identification, to:

Cost:

• Free for genealogical records, $10.00 for a certified copy
