• Per Utah law, Vital Records are available to immediate family members or to persons with written permission from the immediate family.
  
• If you meet these requirements, download this application for a certified copy of a death certificate from the Utah Department of Health.
• Complete the above application and mail it along with any applicable fees, and a copy of your government issued photo identification, to:

Records no longer maintained by the Office of Vital Records and Statistics are found at the Division of Archives & Records Service (see link below). This includes:

• Death records 50 years old or more
• Birth records 100 years old or more
• Marriage and divorce records 75 years old or more.
