Pennsylvania Free Death Records

• Pennsylvania is a "closed record state," which means death records can only be provided to those who are eligible.
• In order to request a death certificate in Pennsylvania, you must be:
• Legal representative of decedent's estate
• Immediate family member
• Extended family member who indicates a direct relationship to the decedent
• Power of Attorney (Please note that a Power of Attorney document is no longer valid upon the death of the individual)
  

• If you are eligible, simply download this Application for Certified Copy of Death Record from the Pennsylvania Department of Health.
• Complete the above application and mail it, along with any applicable fees and a copy of your government issued photo identification, to:

Notes:

• Fee may be waived if the decedent was an Armed Forces member or related to the Armed Forces member.
• Mail requests are processed in approximately twelve to fourteen weeks from the date of receipt.
