Oklahoma Free Death Records

• Oklahoma is a "closed record state," meaning that death records are not available to the general public.
• To be eligible to request a death certificate from Oklahoma, you must be able to describe your relationship to the deceased and purpose for needing a copy.
• If you are eligible, all you have to do is download this Application For Search And Certified Copy Of Death Certificate from the Oklahoma State Department of Health.
  
• Complete the above application and mail it, along with any applicable fees and a copy of your government issued photo identifaction, to:

Notes:

• For requests by mail, allow at least 4 weeks for the request to be processed.
• Oklahoma began filing death records in October of 1908. It was not mandatory, however, that these records be filed until 1917.
• Because death records were not required in years past as they are today for identification, the filing process in many years prior to 1940 are sketchy.
