How to Apply:
- If the death occurred more than 50 years ago, New York considers it to be "genealogical research," which means that anyone can apply to view a death certificate.
- Due to the size and population of New York City, the State of New York divides the records by deaths that occurred within the New York City limit, and deaths that occurred outside the city limits.
- To be eligible for death records less than 50 years old, you must meet certain requirements (see Notes below).
- For deaths that occurred in New York City:
- Prior to 1949, download this form and mail it, along with any fees and a copy of your government issued photo identification, to:
