  • Proof of relationship is not required if you are asking for a certification, which is an uncertified informational copy of the vital record not valid for establishing identity or legal purposes.
  • Download the Death Certificate Application Form
  • from the New Jersey Department of Health and Senior Services.
  • Complete the above form and mail it, along with any applicable fees, and a photocopy of your government issued photo identification, to:
    • New Jersey Bureau of Vital Statistics and Registration 
      Customer Service Unit – Non-genealogical Requests

      Trenton, NJ 08625-0370


Notes:

  • The Office of Vital Statistics and Registry registers vital events and maintains death records back to 1901.
  • If you are looking for death records prior to 1901, you will need to contact the New Jersey Division of Archives.
  • Proof of relationship is only required if you need to obtain a certified death certificate, anyone may apply for an un-certified certificate (used for genealogical research).
