How to Apply:
- Nevada is a "closed record state," which means only certain people are eligible to request certified death records.
- A qualified applicant is defined as the registrant, or a direct family member by blood or marriage, his or her guardian, or his or her legal representative.
- If you fit the above requirements, you can download the Application for Death Certificate from the Nevada Department of Health and Human Services.
- Complete the above application and mail it, along with any applicable fees, and a copy of your government issued photo identification, to:

Notes:
- If you need to correct a death certificate, download the Affidavit for Correction of a Record from the Nevada Department of Health and Human Services.
- The Nevada Department of Health and Human Services is the legal custodian of birth and death certificates filed in the state of Nevada from July 1, 1911 to the present. For events prior to 1911, records are usually held in the county Recorder’s office where the event occurred.
