• Nebraska is a "closed record state," which means you cannot obtain a death record without proper purpose. Click here to find out if you are eligible.
• If you are eligible, download the Death Certificate Application from the Nebraska Department of Health and Human Services.
  
• Complete the above application and mail it, along with any applicable fee and a copy of your government issued photo identification, to:

Notes:

• If you notice an error on the death certificate you obtain, fill out a Application for Amendment from the Nebraska Department of Health & Human Services.
