Minnesota Free Death Records

• Minnesota is among the few states that indexes all death records online, and makes them available to the public (see link below).
• Death records from 1908 - 2001 are available online. If you need a more recent death certificate, you will need to apply by mail.
• To find a death record from the index, you will need to know the person's first and last name. If you know the year or decade that the person passed away, or the county, this will help narrow down your results.
• If you want a certified copy of a death record from the index you can order them directly from the website for $9.00.

• The Minnesota Death Certificates Index is a free service provided by the Minnesota Historical Society. If you find their service useful, make sure to donate and support them!
• If you need a Minnesota death certificate or death record from 2002 or later, you'll need to fill out the Death Certificate Application Form from the Minnesota Department of Health
• Mail your completed application, along with any applicable fees and a copy of your photo identification, to:
