• Download the Application for a Certified Death Record from the Michigan Department of Community Health.
• Complete the above application and mail it, along with any applicable fees and a copy of your government issued photo identification, to:

Notes:

• Search fees are non-refundable.
• The Michigan Vital Records office has records of deaths that occurred in
  Michigan and were filed with the state since 1867.
• Anyone is eligible to request a copy of a Michigan death record if the application is
  completed and signed and the required fee is paid.
• If you need your request expedited, address the mail to "Vital Records RUSH."

Cost:

• $26.00 for a one year search, $12.00 for each additional year needed
