• Download the Mail Order Form from the Massachusetts Department of Health and Human Services.
• Fill out the above form and send it, along with a copy of your government issued photo identification, and any applicable fees, to:
Registry of Vital Records and Statistics

Notes:

• Most, but not all, death certificates are available to the public. Massachusetts is not a closed record state.
• If you need the record expedited, it will cost significantly more.
• Make your check or money order payable to "The Commonwealth of Massachusetts"
