Louisiana Free Death Records

• Death records that are more than 50 years old are available to the public online, at the Louisiana Secretary of State website (see below for link).
• For death records more recent than 50 years ago, you'll need to download the Application to Obtain a Certified Death Certificate from the Louisiana Office of Public Health.
  
• Download and fill out the above application and send it along with a copy of your government issued photo identification, and a mail order or check for any applicable fee, to:

Notes:

• Louisiana is a "closed record state." This means that birth and death certificates are not public records. All requests for death certificates must include proper identification.
• If you are filling out the request by mail, please allow 4-6 weeks for delivery.

• Free (for death records more than 50 years old), $7.00 otherwise
