  • Kentucky is among several other states in the country that makes their death records available for free, online, to the public. All you have to do is register for the "KY GenWeb Project," which is funded entirely by supporters.
  • You'll need to know the First and Last name of the deceased, the place of death, and the residence at the time of their death.
  • Death records from 1911 - 2000 are available online, if you need a more recent one you can download the Death Certificate Application Form from the Kentucky Cabinet for Health and Family Services.
  • Mail the above form, along with a copy of your photo identification and any applicable fees, to:

    • Notes:

    • Death certificate requests by mail will take up to 30 working days to fulfill.
