• Download Form 49606 from the Indiana State Department of Health
  
• Complete the above application and mail it with a photocopy of your government issued photo identifiaction, and any applicable fees, to:

  Vital Records, Indiana State Department of Health
  PO Box 7125
  Indianapolis, IN 46206-7125

• Only parents, spouse / life partners, adult children, grandparents, siblings, aunts / uncles, direct descendents, nieces / nephews and other family members may apply.

Notes:

• A photocopy of your government issued photo identification and additional documents may be necessary to prove your eligibility to receive the certificate. Click here for a full list of requirements.
• Death records in the ISDH Vital Records office begin with 1900. Prior to 1900, records of death are filed only with the local health department in the county where the death actually occurred.
