• Complete the above application and mail it, along with a check or money order, to:

• If you are looking for a death record from 1916 - 1950, Illinois offers an online database for free that anyone can access. Check out the link at the bottom of the page under "Websites"!

Notes:

• If you are looking for death records from 1916 - 1950, they can be accessed for free from the Illinois Department of Public Health website (see below).
• Regular mail requests are processed, on average, between 8 - 10 weeks.
• Illinois law states that death records may be issued only as listed below:
• Upon the specific written request for a uncertified/certified copy by a person, or his duly authorized agent, having a genealogical record (record must be over 20 years old), personal or property right interest in the record.
• If you fall under that eligibility, you will need to indicate your relationship to the person registered on your application.

• Free (for death records 1916 - 1950), $17.00 otherwise
