• Download and complete the Application for Copy of Death Certificate from the Connecticut Department of Public Health
  
• Mail the completed form, along with a postal order for any of the applicable fees, to:

Notes:

• The State of CT is has a turnaround time of approximately six (6) weeks
• For a quicker turnaround time you may direct your request to the town where the event occurred.  For a town address listing and telephone numbers, click on the following link for the  Connecticut Town Clerk and Registrar Directory.
• If you are requesting a death certificate for a death that occurred after July 1, 1997, the social security number of the decedent will not be included on the certificate that you receive unless you are an immediate family member of the decedent, the executor of the decedent’s estate, a party specified on the death certificate such as the informant, funeral director/ embalmer, or certifying physician, or other persons as authorized by Department of Public Health.
