• Download and complete the Death Certificate Application from the Colorado Department of Public Health and Environment also available in Spanish
• Mail the application along with any necessary fees to:

Notes:

Copies of documents needed to prove eligibility vary depending on your relationship to the deceased. For example:

• Current spouse must be listed on the death certificate
• Ex-spouse must present proof of direct & tangible interest (i.e. social security number, insurance policyu)
• Parents must be listed on the death certificate
• Step parents must provide a marriage certificate that proves relationship to the parent of the deceased who is listed on the certificate.
• Children, grandchildren and great-grandchildren must present a birth certificate showing the relationship to the deceased.
