Arkansas Free Death Records

• Download the Death Certificate Application from the Arkansas Department of Health
  
• Submit the above application along with a copy of your government issued photo identifaction,
• Full name of the deceased
• Date of death
• County or city of death
• Name of the funeral home (optional)
• Requester's relationship to the deceased
• Requester's reason for requesting the certificate
• Please note if this was a stillborn infant (See "Certificates of Birth Resulting in Stillbirth")
• Cash, check, or money order made payable to Arkansas Department of Health

Notes:

• Arkansas allows release to specific persons who are related to the registrant, his/her designated representatives, academic research groups, and to persons who can demonstrate that they have a right to obtain the record.
